If you are on per diem: You can register for both conference fees and meals but pay for them with separate checks. Please enclose both checks with this form. If you need separate receipts for conference fees and meals, please print two copies of this form and fill them in separately—one with conference fees but no meals, and the other with meals but no conference fees. Then enclose separate checks and mail both forms and both checks together.

Banquet and catered lunches: Please note that if you don’t reserve and pay for the banquet and catered lunches in advance, you probably won’t be able to buy them at the door—we have to provide the hotel with 48 hour notice for the number of meals, but onsite registration provides less than 24 hour notice. Hence, we’ll have only a very limited number of meal signups at onsite registration and these will be first-come, first-served (sorry!)
