| The downloaded form should open in "Acrobat Reader" which is pre-installed in most computers sold in the past several years. In case it doesn't open by clicking on the file icon, try finding the word "Acrobat" in your file system, opening the Acrobat Reader application, and then opening the downloaded file using the "Open" menu choice in Acrobat's "File" menu. If you don't have Acrobat Reader, you can download it for free from www.adobe.com.
Print a copy of the file in Acrobat Reader, fill in both pages of the printout, and snail-mail it to us at the address below. Regular snail-mail is OK as long as we receive it on or before Feb. 5. After that, please use our online registration. Sending your registration and payment WHEN SENDING THE CHECK, PLEASE MAKE IT CRYSTAL CLEAR WHO THE CHECK PAYS FOR so that we can match it to your online registration!
